He was predeceased by: his father Griffin Cook; his brother Glenn Cook Sr.; and his stepfather Leo Olson. He is survived by: his wife Cathy of Erlander, KY; his mother Mildred Olson (Leo) of Kahoka, MO; his children, Adam Cook (Nikki Brinkman) of Burlington, KY, Odie Cook (Tracy Schmitz) of Erlanger, KY, Samantha Williams (Joseph) of Erlanger, KY and Mark Cook of Paducah, KY; his grandchildren, Corban Stilts, Addyson Cook, Charlette Cook, Sydney Humphrey, Odie Cook 3rd, Sierra Schmitz, Oakley Cook, Sonoma Schmitz, Abigail Cook, Joseph Williams Jr., Savannah Williams, Sabrina Williams and Owen Cook; and his siblings, Susan Roberts of Wayland, MO and Mike Cook of Keokuk, IA..
