She was predeceased by: her husband Johnny Pack; her parents, Grant Horn and Myrtle Gearheart Horn; her daughter Libby Pack; and her siblings, Mack Horn, Maxine "Bootsy" Horn, Rosemary Brown, Georgia Brown, Donna Helen Young and Bonnie Jo Matheny.
She is survived by: her daughters, Tami Singer of Cincinnati, OH and Teresa Munschauer (Philip) of Bellevue, KY; her siblings, Robert Horn, Ronnie Horn of Hueysville, KY and Connie Sue Prater of Garrett, KY; her grandchildren, Kyle, Ryan, Rachel, Graydon, Jason, Jacquelyn and Samantha; and her great grandchildren, Nova, Connor, Hayley, Eleanor, Alexis, Addie, Chloe, Rini, Beckett and Carson.
In lieu of flowers the family has requested donations be made Lewy Body Dementia Research.
