She was predeceased by: her husband William Smith; her former husband Fred Gullett; her parents, Wesley Phelps and Georgia Phelps; and her brother Marvin Phelps.
She is survived by: her sons, Scott Smith (Lynn) of Elsmere, Jeffrey Smith (Gail) of Crescent Springs and Steve Smith (Nicole Elliott) of Elsmere; her granddaughter April Johnson; and her great grandchildren, Devin, Josh Justice and Jordan Johnson.
