He is survived by: his wife Linda Reed Cook of Elizabethtown; his children, Keith Dale Cook of Elizabethtown, Rhonda Foutch (Larry) of Hazel, KY, KaSandra Cook of Murray, KY and La'Chelle Maturen of Athens, MI; and his sister Elizabeth Oakley of Paris, TN. He is also survived by nine grandchildren and six great grandchildren.
