He was predeceased by: his parents, Albert Cook and Ruie McFarland Cook; and his siblings, Wesley, Bobby Cook and Joyce Nichols.
He is survived by: his wife Sylvia McCoy Cook; his sons, Tracy Cook (Beverly) and Kelly Cook; his siblings, Betty Robertson, Gladys Hughes, Donnie Cook and Jessie Cook of Bowling Green; and his grandchildren, Daniell Elmore, Nicole Cook, Owen Kelby Cook and Zackary Cook. He is also survived by three great grand children.
In lieu of flowers memorials may be made to the American Heart Association.
