He was predeceased by: his parents, Larcie H. Lewis and Pearlie Mae Miller Lewis; and his brother Roger Dale Lewis. He is survived by: his wife Charlene Hicks Lewis; his children, Jerry Lewis (Rechelle) of Richmond, KY, David Lewis, Jason Lewis (Amy Chipman) and Tonya Lewis of Lexington, KY; his siblings, Clyde P. Lewis (Brenda) and Connie Mae Roatcap of Pierceton, IN.; and his grandchildren, Cheyenne Lewis, Christopher Lewis, Mitchell Lewis, Shawn Lewis, Jacob Lewis (Sydney) and Tyler Lewis.
