He is survived by: his wife Mary Chamberlain of Earlington, KY; his children, Nicole Raymond (Todd) of Suthards, KY and Otis Chamberlain, Jr. of Suthards, KY; his siblings, Veronica Miller of Madisonville, KY, Beverly Simerly of Belvedere, IL, Sharon Bates of Nortonville, KY and Isaac Chamberlain of Cocoa, FL; his grandchildren, James Isaac Chamberlain, Isaac Ray Chamberlain, Brandon Allen Raymond, Haven Lynn Raymond and Gabriel Evan Chamberlain; and his great grandchild Asher Brock Raymond. He is also survived by several nieces and nephews.
