He was predeceased by: his parents, Bobby Middleton and Shelby Stobart. He is survived by: his wife Gina Shocklee Middleton; his daughters, Sydney Middleton (Nature) of Owensboro and Ryley Middleton of Beechmont; his stepchildren, Michael Kennedy (Sarah) of Graham and Nikki Takacs (Josh) of Fransisco, IN; his grandchildren, Jachin Takacs, Kaleb Takacs and Connor Kennedy; and his sister Glenda Lacey of Hawaii.
Family and friends who are attending the visitation or funeral service are asked to wear a face mask for the safety of all those in attendance, in accordance with the governor's mandate.
