He is survived by: his wife Deana Marie Endicott; his children, Austin Tyler Lewis of Williamstown, Samantha Lynn Lewis of Williamstown and Hannah Marie Lewis of Dry Ridge; his brothers, Jerry Alan Lewis of Dry Ridge and Johnny Raymond Lewis of South Carolina; his nephew James Tyson "Goobie" Mitchell; and his grandchildren, Ameliah Marie Lewis, Emerlynn Love Mayhan and Dalyn Mayhan.
