He was predeceased by: his parents, Steve O'Banion and Alma Frances Gray O'Banion; his sons, Bryan "Bernie" Stephen O'Banion and Jason "Jake" Patrick O'Banion; and also Betty Eloise Wright.

He is survived by: his wife Brenda Kaye O'Banion; his grandchildren, Lilly Amber Rae O'Banion of Dry Ridge, Steven Patrick O'Banion (Brandi) of Williamstown and Stephanie Good of Williamstown; his great grandchildren, Aksel, Jaislyn, Millie, Lane and Garridyn; and his brother David Kline O'Banion of Sioux Falls, S. D..
