He was predeceased by: his father Larry Perkins. He is survived by: his mother Phyllis Perkins; his sisters, Paula Kim Kelley of Florida and Nita Jan Perkins of Dexter, KY; his nieces, Sabrina, Amy, Lori and Savannah; his nephew Michael; and his great-nieces, Tori and Carmen. He is also survived by several Aunts & Uncles.
