He is survived by: his wife Carolyn Springfield-Forbes; and his daughters, Melinda Martin (Edward) of Dawson Springs, Rhonda Ann Putty (Larry) of St. Charles, KY and Vicky Woolhouse (Kevin) of Wheeler, WS. He is also survived by eight grandchildren; five step grandchildren; several great-grandchildren; several great-great grandchildren; and several nieces and nephews.
