He is survived by: his wife Nancy Ann Thomas Mathes of Beulah, KY; his son Carmon Dale Mathes of Beulah, KY; his grandchildren, Nathan Tapp (Madeline) of Manitou, KY and Brooklyn Jones (Heath) of Nebo, KY; and his great grandchildren, Leland Tapp and Mason Jones.
