He was predeceased by: his parent Kelly Mullins; and his sister Otillia Mae Hensley. He is survived by: his mother Alice Romine Plantz of Middleport, OH; his daughter Annabelle Pyott Perez of Warsaw, IN; and his brother Kenneth Mullins (Roxana). He is also survived by grandchildren and a number of nieces and nephews.
