He is survived by: his parents, Betty Joe Robbins Cook and Chester Cook; his wife Lisa Underwood Cook of Danville; his son Christopher Ray O'Neal Cook (Holly) of Danville; his granddaughter Emma Kate Cook; and his siblings, Scottie Cook (Lesa), Danny Cook of Stanford and Connie Tankersley (Scot) of Stanford.
Visitation was held on Monday, March 17th 2025 from 11:00 AM to 1:00 PM at the Indian Hills Christian Church (516 Grabruck St, Danville, KY 40422). A memorial service was held on Monday, March 17th 2025 at 1:00 PM at the same location.
