He was predeceased by: his parents, Herbert Lewis and Naomi Combs Lewis; and his brother Tony Lewis. He is survived by: his girlfriend Lana Fritz of Virginia; his sons, Dalton Lewis of Harlan and Evan Lewis of Georgetown; his siblings, Leon Lewis (Missy) of Cumberland, Darlene Mandrell (Rick) of Cumberland and Loretta Simpson of Cumberland; and his nieces and nephews, Candace, Erica, Jessie, Stacey, Cody, Keli, Doodle, Caden and Matt.
