He is survived by: his wife Catherine Carroll Avery; his children, Patrick Avery (Bekah) of Cub Run, Marilyn Johnston of Upton and Cindy White (Troy) of Munfordville; his grandchildren, Shaun (Kaitlin), Katie (Brent Smith), Rusty Dale, Wyatt and Cooper; his great grandchildren, Abby and Emily Smith; and his siblings, Gerald Avery and Margie Logsdon.
The family request that memorial donations be given to the Pine Grove Cemetery.
