He was predeceased by: his parents, John Harvey Cook, Jr. and Opal Marie Sisk Bennett; and his brother Bobby Joe Cook. He is survived by: his children, Travis Cook (Sandy) of Morgantown, KY and Shannon Johnson of Clay, KY; his stepchildren, Allen Groves (Tracey) of Crofton, KY and Jamie Tucker of Hopkinsville, KY; his brother John O. Cook (Linda Mart) of Crofton, KY; and his sister-in-law Maria Cook. He is also survived by 11 grandchildren; and 1 great grandchild.
