She was predeceased by: her husband Michael L. Tompkins. She is survived by: her children, Michael P. Tompkins (Mary Ann), Paul Allyn Tompkins (Leo) and Thomas J. Tompkins (Heather); her grandchildren, Ian, Alex, Marc, Shawn, Stuart and Eric; and her sisters, Jackie (Jim) and Christie.

The family kindly requests donations be made to the Alzheimer's Association in Phyllis's memory, as a tribute to her.
