He is survived by: his daughter Tracy Campbell (Frankie) of Cox's Creek; his grandchildren, Mackenzie Cook, Tristun Cook, Scottie Cook, Kaydence Hatfield, Brisun Cook, Lola Campbell, Stella Campbell, Ava Campbell, Ex-Wife and Lois Hall Cook; his mother Carolyn Ballard; his siblings, Donna Bowman, David Cook, Rose Spalding, Mona Carey, Annette Curtsinger, John Cook and Chris Cook; and also Troy Cook (Brittany) of Bardstown. He is also survived by several nieces, nephews, cousins and friends.
