He was predeceased by: his parents, Stanley Smith and Mary Jo Quiggins Smith; his son David Smith; his stepson Joseph Carden; and his siblings, Harold Sarver, Steve Sarver, James Robert Smith and Margaret Carol Sarver.
He is survived by: his wife Margaret Bolen Smith of Clarkson; his son Leslie Smith Jr. of Clarkson; his stepchildren, Richard Elliott of Clarkson, Jimmy Carden of Fordsville, KY, Melinda Schultz of Falls, Christy Schultz (Jared) of Hartford, KY and Carol Huffman of Paducah, KY; and his sisters, Rosemary Fentress of Louisville, KY, Linda Harding of Louisville, KY, Betty Jean Clemons of Clarkson, Faye Blair of Clarkson and Patricia Franks of Caneyville, KY.. He is also survived by nine grandchildren, two great-grandchildren.
