He was predeceased by: his wife Kathy Jean Self. He is survived by: his son Derek Self (Emmi); his grandsons, Slayten Weller Self and Jett Jameson Self; his siblings, Judy Smith, Debra Batliner and Danny Self (Debbie); and his close friend Beth Page. He is also survived by several nephews, nieces and cousins.
David served as the chairperson for the Cave City Cemetery Board and asked that memorial contributions be made to Cave City Cemetery 105 Duke Street Cave City, Ky 42127.
