He is survived by: his wife Tina Hoffman; his daughters, Rachel Blackburn (Paul) of Owenton, KY and Kelly Simmons of Carrollton, KY; and his siblings, Barbara Wolfe of Mississippi, Wilbur Hoffman of Ekron, KY and Steven Hoffman of California. He is also survived by four grandchildren.
