He is survived by: his wife Lana Vaughn of Carlisle; his brother Alan Cook of Portland, Oregon; his half-sister Lara Luna of Walnut Creek, California; his daughters, Alicia Cook Bennett (Jeff) of Central Point and Andrea Cook of Boulder Creek, California; and his granddaughters, Brittaney Bentley and Daytona Cook.
