He was predeceased by: his parents, James Collins and Ruth Collins. He is survived by: his son Todd Carson (Vanessa) of Beattyville, KY.; his brothers, Jeff Collins of Utah and John Collins of Campton, KY.; and his niece Heather Trent. He is also survived by nieces, nephews, other relatives, and friends.
