She was predeceased by: her parents, Melvin Roth and Rose Roth (Needleman). She is survived by: her daughter Erin Hazelwood (Terrell) of Campbellsville; her grandchildren, Mia and Noah Hazelwood; and her siblings, Gary Roth of Hawaii and Vicki Zimmerman of North Dakota.
