She was predeceased by: her parents, Edmund Kotecki (Witnesses) and Anna Urbanski Kotecki; her husband Edward Joseph Kozak; and her sister Evelyn Kee-Kotecki. She is survived by: her son Howie; her daughter-in-law Maureen Kozak; her grandchildren, Keith Howard Kozak (Tiffany) and Karen Elizabeth McConnell (Jimmy); and her great grandchildren, Chloe of Campbellsville, Kya of Campbellsville, Kiara of Campbellsville and Cage of Campbellsville. She is also survived by many loving nieces, nephews and cousins, all of Illinois and her brothers and sisters.
