He was predeceased by: his parents, Stanley Keith and Sarah Underwood Keith; his daughter Brianna Michelle Keith; and his siblings, Martha Highland, William G. Keith and Randy Keith.

He is survived by: his wife Cheryl Keith of Cadiz, KY; his children, Adam N. Keith of Louisville, KY, Kristin N. Keith of Louisville, KY and Kali L. Keith of Owensboro, KY; and his granddaughter Khole Marie Daugherty.
