He is survived by: his wife Janice Smallwood Curtis; his sons, Trent Curtis (Maegan) and Josh Curtis (Maggi); his siblings, Shawnna Sharp (David), Earl Johnson Jr. (Pam) and Don Johnson (Sharon); and his granddaughters, Melanie, Natalie Boone and Josie.
