He was predeceased by: his mother Jolene Edwards Perkins; and his grandparents, Ray Perkins, Bill and Reva Young Edwards. He is survived by: his father David Ray Perkins (Lisa) of Buffalo; his children, Asa and Audrey; his siblings, Jessica Bowling (Michael) of New Haven and Joshua Ray Perkins of Buffalo; his stepsister Stacie Hutchison (Michael) of Bowling Green; his grandmother Mamie Warren Perkins of Elizabethtown; and his nieces and nephews, Clayton, Colten, Olivia, Amelia and Lucy. He is also survived by several aunts, uncles, cousins and friends.
