He was predeceased by: his former wife Patty Cook; and his mother Dola Cook. He is survived by: his wife Teresa Cook; his children, Terry Cook (Karen), Brandon Cook (Amy) and Travis Cook (Dorothy); his grandchildren, Devin Cook, Carter Cook, Thomas Cook and Anna Cook; and his siblings, Mark Cook (Rhonda), Norma Schmitz, Sue Jones (Larry), Janice Collins (Roger), Shelly Bryant, Kathy Thacker (Ronnie), Marcella Keane, Estill Cook and Vaughan Cook.
