He is survived by: his wife Theresa Brussell Cammack; his children, Josh Cammack (Stacey) and Jessica "Jess" Medley (Michael) of Bardstown; his grandchildren, J. T., Ryker, Levi, Luke, Zoe and Israel; and his half-sister Judy Hughes of Bowling Green.
The family request that memorial donations take the form of contributions to Leukemia and Lymphoma Society or American Cancer Society.
