Ballard of Phoenix, AZ; her grandchildren, Tanganyika Johnwell (Christopher) of Phoenix, AZ and Jarvis Reddick II of Phoenix, AZ; her brothers, Larry Ballard of Richmond, KY and Randall Ballard (Shelia) of Lexington, KY; her nieces and nephews, Valarie Floyd (Frank) of Richmond, KY, Donna Jenkins (Darrell) of Richmond, KY, Regina Simpson of Richmond, KY, Otis Ballard (Cynthia) of Richmond, KY, Anthony Ballard (Donna) of Richmond, KY and Tony Sweat (Lisa) of Atlanta, GA; her grandson Anthony Smith; her nephew James Smith; and her friend Sandra Harris.
