He is survived by: his wife Maria Smith of Beaver Dam; his children, Brennan Smith of Beaver Dam, Jimmy Smith of Beaver Dam and Jensen Smith of Beaver Dam; his granddaughter Addison Baize; and his siblings, Doug Smith of Owensboro and Tammy Sampson (Todd) of Sturgis. He is also survived by several nieces and nephews.
