He was predeceased by: his parents, Jerry D. Hayse and Vina Lindsey Hayse; and his siblings, Phyllis Hayse, Patricia Kessinger and Eric Hayse.
He is survived by: his significant other Paula Sanders; his children, Jerry David Hayse II of Beaver Dam, Shelta Dawn Hayse of Hartford, Tony Sanders of Beaver Dam, Tinika Sanders (Aaron) of Beaver Dam and Tosha Sanders (Aaron) of Beaver Dam; and his brothers, Larry Hayse (Kathy) of Hartford, Michael Hayse (Holly) of Beaver Dam and Barry Hayse (Linda) of Missouri. He is also survived by cousins, nieces and nephews.
