He was predeceased by: his parents, John Lee Polivick and Louise Polivick (Munsell); and his siblings, Helen Gill, Junior Polivick and Dennis Polivick.
He is survived by: his daughter Cheryl Waldon (Neil) of Kevil; his siblings, Agnes Lindsey of Bardwell and Juett Polivick of Wickliffe; and his grandchildren, Blake Waldon and Brent Waldon.
