He was predeceased by: his daughter Rose Mary Lewis; his parents, John R. "Buck" Lewis and Gladys Hughes Lewis; and his siblings, John Frank Lewis, Jackie Lewis and Clara Louise Lewis.
He is survived by: his wife Rose Marie Lewis of Bardstown; his children, Charles Lewis (Patricia) of Louisville, Linda Lewis of Lexington and Bonnie Lewis of Bardstown; his grandson Zachary Lewis (Megan) of Louisville; and his sisters, Mary Catherine Lewis of Bardstown, Minnie Lewis of Bardstown, Carrie Lewis of Bardstown and Betty Squires of Louisville. He is also survived by nieces and nephews.
