He was predeceased by: his parents, John Blevins and Ada Blevins. He is survived by: his wife Ann Blevins; his sons, Mark Blevins (Sherry) of Bardstown, Scott Blevins (Alison) of Charlestown, Matthew Blevins (Jami) of Bardstown and Daniel Blevins of Louisville; his grandchildren, Sean Blevins, Shelly Blevins-Stratton, Wyatt Blevins and Luke Blevins; his great grandchild Ava Blevins-Stratton; and his siblings, Yvonne Tomacic and Butch Fortin.
