He is survived by: his wife Bonnie Lewis; and his siblings, Roy Lee Lewis (Rose), Mary Catherine Lewis of Bardstown, Minnie Lewis of Bardstown, Clara "Louise" Lewis of Bardstown, Carrie Lewis of Bardstown and Betty Squires of Louisville.
