He was predeceased by: his parent Hallie Lewis Holbert. He is survived by: his wife Janette McCarty Lewis; his stepchildren, Rachel Gullett (D. D.) and Rick Smith of Ashland; and his stepgrandchildren, Noah Gullett, Ira Gullett and Bethany Gullett.
