He was predeceased by: his mother Rebecca. He is survived by: his father Donald D. Cook; his wife Susan; his daughters, Jennifer Bosley (Hank) and Jessica Racke (Brad); his siblings, Dale Cook (Jo Ann), Pamela Kramer (Todd) and Charlene McFarland (Mac); and his grandchildren, Jazzlynn Simon-Cook, Dominic, Jacob, Stella Bosley and Beatrice Racke.
