She was predeceased by: her stepfather Mel Collins; and her grandparents, Jerry, Shirley Cox, Bob Perry and Butch Clark.
She is survived by: her parents, Susan Collins (late Mel Collins) and Bobby Perry; her stepmother Terri Kemerly; her siblings, Amber Masters (Rick), Jarod Collins (Melissa), Matt Collins (Dana), Jenn Moellman, Jess Harmon and Tara Perry; her grandmother Juanita Clark; her nieces and nephews, Brennan, Morgan, Amelia, Jacob, Jae, Finlay, Nathan, Phoenix, Alexander, Carson, Collin, Hayley, Logan, Isaac and Soloman; and her pets, Kylo, Kit, Flynn and Poppy. She is also survived by many aunts, uncles, and other close family and friends.
Visitation was held on Thursday, January 2nd 2025 from 5:00 PM to 9:00 PM at the Chambers & Grubbs Funeral Home - Independence (11382 Madison Pike, Independence, KY 41051). A funeral service was held on Friday, January 3rd 2025 at 10:00 AM at the same location.
