He is survived by: his wife Susan; his children, Luke Collins and Staci Slater (Adam) of Yates Center, KS; his grandchildren, Callan Slater, Madelin Slater, Avery Eakin and Maverick Eakin; his father Robert "Ike" Collins of Piqua; his siblings, Randy Collins (Sandy) of Piqua and Kris Wallace (Chris); and also Whitney Sorenson.
