He was predeceased by: his parents, Chester William Cook and Dorothy L. Cook (Connett); and his wife Melba Jo Cook. He is survived by: his son Bob Cook (Kay) of Wellington; his grandchildren, Chrissy Donelson (Garren) of Richmond, TX and David Cook (Maeve) of Marshall, MO; his stepgranddaughter Carolyn Parson of Andover, KS; and his great grandchildren, Anakin, Rita, Lucas, Theren, Joey, Jeremiah and Jacob.
A Memorial has been established with the Alzheimer's Association.
