He was predeceased by: his parents, John William and Elizabeth Josephine Collins (Nutt); and his brother John Scott Collins. He is survived by: his wife Mary Ellen of Maine; his daughters, Heather Peek (Brad) of Overland Park, KS, Holly White (Jeff) of Olathe, KS and Haley McCammon (Chris) of Elkhart, KS; his grandchildren, Sydney, Banks, Kennedy, Taylor, Hayden and Chase; his brother William "Billy" Jesse Collins; and his sister-in-law Patti Collins of Omaha, NE. He is also survived by relatives and many friends.

In lieu of flowers, memorial contributions may be made to the following charities that were close to Jerry's heart: Smile Train, Samaritan's Purse, or St. Jude's Children's Research Hospital.
