He was predeceased by: his parents, Robert McIntosh and Jill McIntosh (Hunt); his sister Ena Cordt; his children, Richard Ward and Bricky McIntosh; and his granddaughter Margaret.
He is survived by: his wife Janice McIntosh (Jones); his sons, Russell (Michelle) and Steven (Rebecca); his stepchildren, David Juiliano of Kansas City, MO and Teresa Dunn (Sean); and his brother Edward McIntosh (Laurie) of Ada, OK.
