She was predeceased by: her parents, Rudolph Polak and Julia Polak (Sterba); and her brother Richard Polak (Sandra). She is survived by: her husband Dennis Piroutek; her sons, Brad (Julie) of Reno, Russ (Stacia) of Belleville, KS and Jeff of Gardner, KS; her brother Jim Polak (Sue) of Wichita, KS; and her grandchildren, Nicholas, Hayley, Zack and Duncan. She is also survived by other nieces, nephews, cousins and friends.
Memorials may be given to the Smith Center United Methodist Church or Smith Center Heath and Rehab and can be sent in care of the funeral chapel.
