She was predeceased by: her parents, Sam Perkins and Inez Perkins (Harp); her husband George Hazel; and her sisters, Elsie Lee Howland and Peggy Marriott.
She is survived by: her sons, George Hazel (Marina) of Seattle, Robert Hazel of Denver, Richard Hazel (Tammy) of Calico Rock and Bill Hazel (Brandie); and her siblings, Lucille Dawson of Grenada, Keith Perkins of Pueblo West, Patty Rees (Mark) of Lamar, Sandra Howard (John) of Canon City and Jackie Mulberry of Pueblo, Colorado. She is also survived by many grandchildren and great-grandchildren.
