He was predeceased by: his parents, Jack E. Booth and Ruth A. Booth (Mahurin); his grandson Orion Booth; his sister Marcie Haws; his niece Liberty Bonney; and his brother-in-law Fereidoun Shokouhi.
He is survived by: his wife Jody Bay; his children, Michelle Casey (Vance) and Joshua Booth; his siblings, Ellen Bonney (George), Shirley Shokouhi and Miles Booth; and his brother-in-law Doyal Haws. He is also survived by five grandchildren; two great-grandchildren and many nieces and nephews.
