She is survived by: her husband Steve Ptacek of Minneapolis, KS; her children, Doug Betts (Deana), Ally, Lauryn, Tyler, Kim Shorb (Tony), Trenton, Bristol, Seth, Morgan, Elysia, Michelle Johnson (Mark), Halle, Kya, Hunter, Chris Ptacek, Brendyn, Skyler of Bennington, KS, Sarah Ptacek (Shannon Johnson), Grant, Addi, Harper and John Ptacek of Bennington, KS; and her brothers, Lester Ullom (Theresa) of Beloit, KS and Larry Ullom of Ionia, KS.
Memorials may be given to St. Jude's Children's Hospital.
